Complete Denture Fabrication Using Neutral Zone Technique: A case report

Dentures are removable prosthetic devices that replace missing teeth and their adjacent tissues. The fabrication of complete dentures can be challenging, as they must fit comfortably and function properly. One approach to complete denture fabrication is the neutral zone technique, which aims to create a natural relationship between the denture and the oral tissues. Here we present the case of a 67-year old woman who presented to us asking for a new set of complete dentures since her old ones worn out. She was completely edentulous and was observed to possess a severely atrophied mandibular ridge. We employed the neutral zone technique for construction of her dentures and, after a follow-up period of around 1 week, the patient was satisfied with the results and continuing with her daily activities without any complications at the moment.

Background:  In natural and artificial dentitions, the plane of occlusion plays a significant role in fulfilling the criteria of function and aesthetics. It is essential to determine the orientation of the plane of occlusion to restore or re-establish the lost occlusal plane as it was initially possible.
 
AIM: To determine whether a similar correlation exists between the Porion Nasion Anterior Nasal Spine (PoNANS) angle and the occlusal plane-Frankfort horizontal (FH) plane angle.
 
Materials and Methods: For this in vivo study, twenty healthy dentate individuals aged between 20 – 40 years with Angles class I molar relation having good temporomandibular joint and no congenital or development deformities were selected. Digital lateral cephalograms of the selected individuals were obtained. Cephalometric analyses performed with AUTOCAD software and reference points [Porion (Po), Nasion (N), Anterior nasal spine (ANS), Orbitale (Or)] were marked. PoNANS and occlusal plane‑FH plane angles were derived from marked reference points. Graph plotted with resultant values determining the X axis (PoNANS angle) and Y axis (occlusal plane‑FH plane). The correlation coefficient was calculated to determine the correlation between the Porion Nasion Anterior Nasal Spine (PoNANS) angle and the occlusal plane-Frankfort horizontal(FH) plane angle. Later, a t-test was applied to determine the significance of the correlation coefficient. Regression analysis was performed, and the equation was derived with the help of which the value of 'Y' can be obtained when the value of 'X' is known.

Digital complete denture versus conventional complete denture. A systematic review

Computer engineered complete dentures (CECDs) or digital dentures have been increasingly being fabricated by dentists these days. Enhanced clinical outcomes are only one of the many benefits of computer-aided design and computer-aided manufacturing (CAD-CAM) technology's introduction to clinical and dental laboratory procedures.
The objective of this systematic review was to compare and summarise the findings of clinical research pertaining to CD manufacture in terms of the distinctions between CAD-CAM technology and traditional approaches.
Medline/PubMed, Cochrane Library, SciELO, Web of Science, and Embase were the databases searched for this systematic review was conducted in accordance with the criteria established by the Cochrane Collaboration and in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses, PRISMA 2020 guidelines. Based on the results of this systematic review, conclusions were drawn and presented.

The natural consequences of ageing can lead to edentulism as well as the loss of muscle tonicity of the face. This can cause unesthetic appearance, functional impairment and thus disturbed psychosocial life. Sunken cheek is one of the major findings of edentulism. It can be managed with conventional maxillary and mandibular denture prosthesis. However additional support may be required in special cases. Cheek plumper prosthesis provides the extra support and restores the facial esthetics; thereby enhancing the self esteem of the patient. Comprehensive evaluation of peri implant bone loss in patients receiving mandibular implant supportedover dentures- A cone beam computed tomography based clinical (Original Research) Study

Background and Aim: Crestal bone is a routine phenomenon around newly installed implant in the jaw bone. It shows different pattern as per patients habits and oral hygiene. Many clinical approaches have been discussed over the years to deal with this issue. Severe bone loss can eventually lead to implant mobility and exfoliation. The sole
endeavour of present study was to assess peri-implant bone loss in implant supported over dentures patients. It was completed utilizing cone beam computed tomography. Materials & Methods: Firstly, twenty new completely edentulous patients were selected from the regular opd of the department. Complete dentures were fabricated by usual manner. Two implants were decided to be placed in the bilateral canine regions in the mandibular arch. Standard osteotomy procedures were utilized for it. Authors planned to evaluate crestal bone losses around these implants. All four surfaces (mesial, distal, buccal, lingual) of implants were analyzed for bone loss at different time intervals. Bone losses were estimated after two months, four months and six months. Group one, group two and group three implants included in which crestal bone loss around all surfaces evaluated after two
months, four months and six months respectively. Cone beam computed tomography was used to calculate bone levels around each implant at the time period of two months, four months and six months. Informed consent was taken from the patients. Statistical Analysis and Results: Statistical analysis was done by statistical software Statistical Package for the  Social Sciences (SPSS). The resultant data was sent to suitable statistical tests to achieve p values, mean, standard deviation, standard error an 95% CI. P ≤ 0.05 was considered as statistically significant. All patients were further separated into five age groups. 2 patients were selected in the age range of 45-52 years. P value was not significant here. 4 patients were noticed in the age range of 52-58 years. P value was not significant for this age group. Total nine patients were identified in the age range of 59-65 years. Level of significance evaluation by pearson chi-square test [for group I and group III] shown significant values of p for all four studied surfaces [mesial, distal, buccal, lingual].Conclusion: Within the limitations of the study, authors concluded that there was obvious crestal bone loss around the implant surfaces. These losses were identified in all three time intervals of study. Relative crestal bone loss was minimum in the first two months on implant placement and maximum in the first six months on the implant placement. Authors also noticed significant differences in bone losses in first two and first six month of implant placement. However, CBCT assessment also verified that this relative increase of bone loss in six months was very minute.

Hinge axis is defined as an imaginary line passing through the two mandibular condyles around which the mandible rotates without translatory movement. Face bow helps to relate the arc of closure or hinge axis of the mandible to the cranium. It records the opening & closing path of movement. Face bow transfer is an integral part in analyzing & studying the occlusion. Face bow record helps in securing anteroposterior positioning of cast in relation to condyles. It also helps in achieving the exact anteroposterior or vertical positioning of cast in relation to condyles.

"Evaluation of Effect Of Dietary Advice Along With New Complete Dentures On Nutritional Status In Edentulous Patients"- An In Vivo Study

Background: The relationship between the oral health and nutritional status in the elderly is complex and controversial. It has been suggested that poor oral health and a decreased ability to chew have a profound influence on food selection, diet and thereby the nutritional status of the individual. Edentulous individuals consume diet that is lower in fruits, vegetables, protein and fiber as compared to dentate individuals due to decrease in ability to chew. If or not a combination of dietary advice along with prosthetic rehabilitation improves dietary intake is still not very clear. So, we are aimed to give dietary advice along with prosthetic treatment and thought of evaluating their nutritional status through OHIP-EDENT, Mini Nutritional Assessment test and serum ferritin levels at baseline, 6 months and 12 months.
Method/Outcome: A type of observational study in which 25 patients will be recruited after seeing into the inclusion and exclusion criteria and then these patients will be given complete denture prosthesis along with nutritional advice and the outcome will be assessed using OHIP-EDENT, Mini Nutritional Assessment Test and Serum Ferritin levels at baseline six and nine months. Expected Results: Simple dietary advice that can be implemented by a dentist would be more practical in clinical practice than tailored dietary counselling. So, giving simple nutritional advice in combination with new complete denture would improve nutrient intake among edentulous individuals

PREVALENCE OF TYPES OF SECONDARY IMPRESSION TECHNIQUES USED BY DENTISTS IN COMPLETE DENTURE PATIENTS- A RETROSPECTIVE STUDY

Impression making is an important part of replacement of the teeth. In complete dentures, secondary impression techniques play a major role in the fabrication of the prosthesis, as they help in recording accurate borders and extension on the impression. Recording the secondary impression plays a major role in the retention, resistance and support. The aim of this study was to assess the prevalence of types of secondary impression techniques used by dentists in complete denture patients. This was a cross sectional study containing sample size of 200. This study was conducted in a university setting. Data was collected from a digital case sheet record and categorised into mucocompressive, mucostatic and selective pressure techniques. Data was then entered in MS Excel Sheets and statistically analysed using IBM SPSS Version
20.0 and graphically represented. Selective pressure impression techniques (42.3%) was the most prevalent type used among dentists. Impression techniques play a major role in the fabrication of denture for its retention, resistance and support.
