COMPARISON OF ATRACURIUM AND CISATRACURIUM WITH REGARDS TO THEIR POTENCY, ONSET TIME OF NEUROMUSCULAR BLOCKADE AND RECOVERY IN PATIENTS UNDERGOING SURGERIES UNDER GENERAL ANAESTHESIA

Introduction: Neuromuscular blocking drugs were first used in anaesthesia and surgery in 1942, ushering in a new era. Succinylcholine, a depolarizing muscle relaxant, was first introduced in 1952, and it quickly became widely used due to the drug's swift action and ultrashort duration of action. However, it was found to cause several undesirable side effects. Hence, efforts have been concentrated on finding the optimal non-depolarizing muscle relaxant with a quick onset and optimal conditions for intubation.
Aim: To compare Atracurium and Cisatracurium with regards to their potency, onset time of neuromuscular blockade and recovery in patients undergoing surgeries under general anaesthesia. Material and methods: We conducted the study in 60 patients of ASA I or ASA II category scheduled for elective surgery under general anaesthesia. Group A (n=30) and Group B (n=30) were given Atracurium (0.5mg/kg) and Cisatracurium (0.15mg/kg) respectively. The onset time of neuromuscular blockade, intubating conditions, duration of neuromuscular blockade, hemodynamic parameters, and presence of any side effects were noted.
Results: The mean onset time of neuromuscular blockade for Group A and Group B were 256.97±8.23 seconds, and 208.70±6.21 seconds, respectively (p value <0.05). In Group A, the conditions of intubation were excellent in 17.24 % of patients and good in 80.65% and while Group B, excellent conditions were observed in 82.76% of patients and good in 19.35% of patients. The mean duration of neuromuscular blockade was 33.83±1.86 minutes in Group A and 59.20±3.40 minutes in Group B (p value <0.05). No side effects were observed in either group.
Conclusion: Cisatracurium (0.15mg/kg) had a statistically and clinically significant faster onset, excellent intubating conditions, and longer duration of neuromuscular blockade when compared to atracurium (0.5mg/kg). However, atracurium was faster in terms of recovery from the last dose of neuromuscular blockade. Both drugs were stable hemodynamically with no side effects

Background:Endotracheal intubation is an integral part of the administration of general anesthesia during the surgery. Neuromuscular blocking agents facilitate general anaesthesia. cisatracurium having lack of histamine release, which provides better cardiovascular stability in comparison to atracurium. To compare of atracurium and cisatracurium onset time, intubation conditions, hemodynamics and duration of block.
Materials and Methods: The present study was a prospective randomize, conducted in the Department of Anaesthesia, Pacific medical college and hospital Udaipur, Rajasthan. Total 70 ASA Grade I/II patients in the age group of 18-60 years posted for elective surgery required general anesthesia were selected in this study.  Patients were divided into two groups of 35 each. Group A patients received atacurium 0.5 mg/kg as loading dose while Group B cisatracurium 0.2mg/kg, The onset time,intubating conditions hemodynamic parameters and duration of action were noted.
Results: Cisatracurium have rapid onset (p<0.001) and long duration of action (p<0.001) and stable mean arterial pressure (p<0.001) and better intubating conditions (p=0.04.).
Conclusion: Cisatracurium provides rapid onset, excellent intubating conditions, long duration of action and good haemodynamic stability in comparision to atracurium.

Comparative study of Equipotent Dose of Cisatracurium and Atracurium in Patients Undergoing Abdominal Laparoscopic Surgeries

Introduction: Cisatracurium is one of the ten stereoisomers of Atracurium and is devoid of
histamine induced cardiovascular effects. The neuromuscular blocking potency of
Cisatracurium is approximately three-fold that of Atracurium. Neuromuscular transmission
was assessed by recording the muscle twitch response to train-of-four nerve stimulation.
Material and Method: This is a prospective study was conducted in the Department of
Anesthesiology, Tertiary care Teaching Hospital over a period of 1 year among 140 patients
aged between 20 and 45 years. Study was performed in 140 patients aged 20-45 years
weighted 40-70 kilograms with ASA physical grading 1 and 2 of either sex scheduled for
elective laparoscopic surgery. Monitoring – ECG, non-invasive blood pressure, pulse
oximetry, temperature and capnography (EtCO2). In operation theatre intravenous cannula of
proper size was inserted into the largest vein on the forearm and an infusion of lactated
ringer’s solution was started at a rate of 5 ml/kg/hr. Induction – inj. Propofol 2 mg per kg.
Patients were divided into 2 groups: Group 1 - 70 patients were given atracurium 0.5 mg per
kg for induction. Group 2 - 70 patients were given cisatracurium 0.1 mg per kg for induction.

Effect of atracurium and cisatracurium as nondepolarising neuromuscular blocking agents: A comparative study

The non-depolarizing muscle relaxant, or the non depolarising muscular blocking (NMBA) agents are known to inhibit the action of acetylcholine, while differing in the onset and the duration of action, potency, metabolic route and side effects.
