Association between serum ferritin levels and elevated blood pressures: A case control study

Introduction: Elevation of serum ferritin levels can result in major health issues. Increased oxidative
stress, inflammation, and endothelial damage are all linked to elevated serum ferritin levels, which in
turn raises the risk of hypertension.
Aim: Serum ferritin levels in patients with high blood pressure and a healthy control group were
measured and compared in this study.
Material and Methods: The present study was a hospital-based observational and analytical study. The
study was conducted over a period of 1 year on 60 patients. The study group comprised of 30 patients
(not diagnosed with hypertension or on any treatment) and the control group comprised of 30 (age and
gender matched) individuals. The levels of serum ferritin were analysed by the chemiluminescent
method.
Result: The results of the present study showed that, the mean value of serum ferritin was 313.17 ±
173.86 ng/ml in study group and 169.50 ± 105.66 ng/ml in control group. The mean SBP was 137.53 ±
14.32 mm/Hg in study group and 121.60 ± 8.09 mm/Hg in control group. The mean DBP was 87.73 ±
8.83 mm/Hg in study group and 71.03 ± 9.56 mm/Hg in control group. The results of the present study
indicate that the difference between the mean serum ferritin and mean SBP, mean DBP among the study
group and control group was found to be significant statistically (p=<0.005).
Conclusion: The current study found a positive association between the mean serum ferritin levels and
high blood pressure among the study participants

A Study of Serum Ferritin in Metabolic Syndrome

Background:The present study was done to determine the association of serum Ferritin
in Metabolic Syndrome as well as to determine the relation between individual
component of metabolic syndrome and number of components metabolic syndrome and
plasma ferritin.
Materials and Methods: It was a Cross-sectional study. The study was carried out at
NRI Medical College & Hospital, CHINAKAKANI, GUNTUR. The study was included
Metabolic syndrome patients diagnosed as per NCEP (National Cholesterol Education
Program) ATP III (Adult Treatment Panel III) (2001) attending the NRI medical
college and Hospital during the study period from March 2021 to March 2022. It is a
Correlational clinical single group study with 102 patients.
Results: In the present study, the mean age group of the study population was 57.38
±8.05. The majority of the study participants belonged to the 51–60 years age group, i.e.,
40.2%, followed by 36.3% in 61–70 years age group. In the present study, the majority
of the study population were male, i.e., 62.7% and females were 37.3%. In the present
study, the majority of the male and female belonged to 51- 70 years age group. In the
present study, overweight was 42%, and obese was 58%. In the present study, the
majority, i.e., 98.4%, had a waist circumference of >90 cm, and among the female
majority, i.e., 94.7% had a waist circumference of >85cm. In the present study, 87.3%
had systolic Blood pressure >130 mmHg, and 84.3% had Diastolic Blood pressure >85
mmHg. In the present study, 62.7% were Hypertensives and were taking regular
medications. 78.4% of the study population were diabetics. The mean duration of
diabetes in the present study was 2.11 ± 0.84. 18.6% had a duration of diabetes <3 years,
3-6 years in 37.3%, and 22.5% had a period being >6 years. 22.5% had 1+ urine
albumin, 2.9% with 2+ urine albumin. 74.5% didn’t show any albumin in urine
analysis. 61.8% had no sugar in the urine. Trances of sugar in urine were identified in
38.2%. Based on the ATP III criteria, metabolic syndrome was classified. 20.6% had 3
components, 28.4% had 4 components, majority i.e. 51% had 5 components identified.
In the present study, a statistically significant association was observed between
metabolic syndrome components and PPBS, Total cholesterol, Triglycerides, HDL, LDL
as the p-value calculated to be <0.05. In the present study, the mean serum ferritin
levels were 126.89 ± 51.77. There was a statistically significant association observed
between components of metabolic syndrome and serum ferritin levels as the p-value
calculated to be <0.05. In the present study, a significant relation was observed between
serum ferritin and waist circumference (r=0.33, pvalue<0.05), Total Cholesterol
(r=0.310 pvalue<0.0001), and LDL (r = 0.326; p value <0.0001).

Simple Febrile Seizure and Iron deficiency Anaemia-Is there any association?.

Febrile seizures occurs mostly in children 14–18 months of age, which overlaps with the maximum prevalence age of iron-deficiency anemia, which is between 1 and 2 years of age.It is well established that iron deficiency may cause some behavioral and developmental problems. The relationship between iron deficiency and febrile seizure is not yet well determined, the present study was conducted to see  the  association  between  iron  deficiency  anaemia  and  simple  febrile  seizures.Methods -This  prospective  case  control  study  was  conducted  in  SMGS  Hospital,  GMC Jammu from January 2018 to  December  2020  after  taking  approval  from  institutional  ethical  committee. Cases  included  children  of  age  group  6  months  to  5  years  presenting  with  simple  febrile  seizure  to  the  casualty  or  OPD  of  this  hospital. For  each  case,  a  control  was  selected  with  similar  age  group  and  same  sex  who  came  for  short  duration  fever  (less  than  3  days) but  without  seizures. deficiency  was  defined  as   per  WHO  criteria After  proper  consent  of  parents  of  the  study  children,  blood  samples  of  the  study  children  were  sent  for  the  above  mentioned  investigations  in  hospital’s  central  laboratory. Samples  were  taken  during  afebrile  period.Results- Total  200  subjects  were  included  in  the  study,  100  in  each  group. Iron  deficiency  anaemia  was  seen  in 46  subjects(46%)  in  case  group  and  in  13(13%)  subjects  in  control  group. The  results  were  statistically  significant {p<0.0001, odds  ratio(95% CI)  5.70(2.82 TO 11.51)}. Conclusion-Iron deficiency anaemia can be considered as a risk factor for febrile seizures and  treatment of iron deficiency anaemia may prevent  occurrence  of  febrile  seizures.

Serum ferritin and liver function test response to oral versus subcutaneous iron chelating agent

Thalassemias are group of inherited autosomal recessive blood disorder. The genetic defect, which could be either mutation or deletion, results in complete absence or reduction synthesis of one of the globin chain that make up hemoglobin. This cause reduction of haemoglobin molecules production ,resulting in decreased of oxygen carrying capacity of the circulation thus causing anemia, the characteristic presenting symptom of the thalassemia. Good chelation therapy and regular blood transfusion protocol have increased the life expectancy.
Objectives : is to compare serum ferritin level and liver function test in a group of patients with beta thalassemia major in response to subcutaneous and oral chelating agent .
Patients and Methods: Prospective analytic study design done on group of 60 patients with beta thalassemia major diagnosed by hemoglobin electrophoresis registered in thalassemia center in AL – Zahra Teaching Hospital for Maternity and Children in AL- Najaf AL- Ashraf , during a period from
1st of February 2019 to 1st of February 2020, aged 2.5 to 17.9 years with serum ferritin levels above 1000 ng/ml and liver transaminases below 5 folds the normal upper limit .
The patients divided into 2 groups, 30 patients were on Deferasirox {Exjade} therapy and 30 patients were on Deferoxamine {Desferal} therapy .
Base line S. Ferritin value as well as LFT (S. GOT , S. GPT , ALP and TSB) value, were taken as base line level and then every (8-12) weeks and were used to assess the changes that occurred in response to subcutaneous and oral chelating agent .
Results : Our study show significant decline in SF (P value <0.05) in both groups after 1 year of treatment with oral or subcutaneous chelating agent. Patients on Exjade show more rate of decrement of serum ferritin (11%) compared to patients taken Desferal who a rate of decrement (6%). Patients on Exjade and Desferal therapy show increment in liver enzymes after 1 year of treatment with statistically significant results, p-value < 0.05 .
Conclusions
We found superiority in oral iron chelating agents (Exjade) to subcutaneous iron chelating agents (Desferal). Serum ferritin level is suitable for long term monitoring as an indicator of efficacy than liver biopsy .

Neurophysiological Assessment And Cognition In Adult Females With Iron Deficiency Anaemia

Abstract: Background: Iron deficiency anemia (IDA) is the most prevalent form of anemia
especially in developing countries. Females of child bearing age are vulnerable for developing
IDA due to menstrual blood loss, dietary deficiency, pregnancy and lactation. IDA is
diagnosed by low blood hemoglobin and serum ferritin levels which correlates with body iron
stores. Of the various biological effects of iron, there are many evidences that iron effects
normal neurological functioning and development. This forms the biological basis for the
behavioral and cognitive developmental delays observed in iron-deficient anemic infants and
children. Reduced levels of blood hemoglobin and serum ferritin has been positively correlated
with low cognitive score and altered evoked potentials in females with IDA. Lack of research
studies to know the effect of IDA on neurophysiological parameters like cognitive
performance, event related potential and brainstem auditory response are little explored in
Indian adult females.
Methods: Cognitive status is assessed using Montreal Cognitive Assessment (MoCA) scale
which is validated testing scale for diagnosis of Mild Cognitive Impairment and
neurophysiological assessment is done using auditory Event Related Potential (ERP / P300)
and Brainstem Evoked Response Audiometry (BERA). MoCA score, ERP and BERA values
will be compared in females with iron deficiency anemia and normal females and also
associated with blood hemoglobin and serum ferritin levels. IDA may impair Cognitive
performance and hearing in adult females which may have tremendous impact on
understanding of speech and problem solving in them.
Hence, the aim of the study is to confirm a relationship between IDA and neuro-cognitive
features using cognitive scale and evoked potentials in adult females.

"Evaluation of Effect Of Dietary Advice Along With New Complete Dentures On Nutritional Status In Edentulous Patients"- An In Vivo Study

Background: The relationship between the oral health and nutritional status in the elderly is complex and controversial. It has been suggested that poor oral health and a decreased ability to chew have a profound influence on food selection, diet and thereby the nutritional status of the individual. Edentulous individuals consume diet that is lower in fruits, vegetables, protein and fiber as compared to dentate individuals due to decrease in ability to chew. If or not a combination of dietary advice along with prosthetic rehabilitation improves dietary intake is still not very clear. So, we are aimed to give dietary advice along with prosthetic treatment and thought of evaluating their nutritional status through OHIP-EDENT, Mini Nutritional Assessment test and serum ferritin levels at baseline, 6 months and 12 months.
Method/Outcome: A type of observational study in which 25 patients will be recruited after seeing into the inclusion and exclusion criteria and then these patients will be given complete denture prosthesis along with nutritional advice and the outcome will be assessed using OHIP-EDENT, Mini Nutritional Assessment Test and Serum Ferritin levels at baseline six and nine months. Expected Results: Simple dietary advice that can be implemented by a dentist would be more practical in clinical practice than tailored dietary counselling. So, giving simple nutritional advice in combination with new complete denture would improve nutrient intake among edentulous individuals
