Factors affecting the surgical outcome of microvascular decompression in trigeminal neuralgia

Background: Microvascular decompression is an established surgical modality for treatment of trigeminal neuralgia with variable long-term success rates depending on numerous patient and disease related factors. 
Methods: A prospective study was done including 31 patients, aged between 18 to 74 years who underwent microvascular decompression for primary trigeminal neuralgia at our institution. Barrows neurological institute pain intensity scale and facial numbness score were used to assess outcome till 1 year follow up period. 
Results: 71% patients were suffering from typical trigeminal neuralgia with involvement of both V2 and V3 divisions being the most common presentation. All patients achieved favorable outcome based on BNI pain and numbness score in the immediate post-operative period however at one year 3 patients had un-favourable pain score. Younger age, longer duration of neuralgia before surgery, involvement of all three trigeminal nerve divisions, pure venous compression and presence of focal arachnoiditis were factors associated with poor outcome. 
Conclusions: MVD is a safe and effective procedure in all age groups including elderly patients with both typical and atypical symptomatology. It is seen to be effective in relieving both arterial and mixed venous neurovascular conflict especially in patients with NVC grade III severity. However, the distribution of pain in all three divisions of trigeminal nerve, pure venous compression and presence of focal arachnoiditis seem to be associated with poorer outcome. 

Prognostic Factors Associated with Mortality in Covid 19 Disease: A Retrospective Single Centre Institutional Study

Background:To evaluate the association of the demographic, clinical sign and symptoms
and laboratory biomarkers in predicting the mortality in COVID-19 infected patients.
Materials and Methods: This was a single centre retrospective observational study
carried out in atertiary care centre of a tribal region of central India, which was a
referral centre for COVID-19.Parameters taken into account were physiological
parameters,symptoms at admission,radiographic findings and laboratory findings.
Results: Out of 103 deaths included in this study 76 were male and 37 females (2:1).
Mean age of the deceased were 54.90 years (18-90 years). The most common symptom
on admission was breathlessness in 92 patients (89.32%). Abdominal symptoms like
diarrhoea and vomiting were associated in 7.7 % of the deceased patients only.Most of
the patients who died were older patients having age 60 years and above (65.04 %).
Most of the patients who died had Spo2 less than 90 % on admission (85.43
%).Lymphopenia (92.23 %) was the most commonly observed finding on blood
investigations done in deceased patients followed by increased CRP level (78.64 %).
Comorbidities were present in 77 patients out of 103 patients (74.75%) included in this
study. Diabetes and hypertension were being the most common comorbidities associated
in these patients.Death was not commonly observed in patients with COVID 19
associated with respiratory diseases.
Conclusion: Identified prognostic factors can help clinicians and policy makers in
tailoring management strategies for patients with COVID-19 infectious disease while
researchers can utilize our findings to develop multivariable prognostic models that
could eventually facilitate decision-making and improve patient important outcomes.

Role of Serum Albumin as a Prognostic marker of mortality and duration of hospital stay among Diabetic Ketoacidosis Patients.

Abstract: Background: Diabetic Ketoacidosis (DKA) is an critical and menacing problem
of Type 1 as well as type 2 diabetes mellitus (T2DM). DKA is associated with high amount
of risk of mortality and morbidity (hospital stay). The prognostic factors of DKA are
important factors affecting the outcome of the patients (as treated or death). S. Albumin is
one of the prognostic factor less studied up till now.
Objectives: To determine Clinical profile of DKA patients: aetiologies, precipitating
factors, biochemical parameters and other complications of T2DM patients and to corelate
serum albumin level with mortality, duration of stay among DKA patients.
Methodology: This cross sectional study will be conducted among 100 diabetic patients
with and without ketoacidosis from May 2019 to May 2021. Biochemical parameters:
glycated Haemoglobin, urine ketones and serum albumin will be gathered to determine the
prognosis. Controls will be adult diabetic patients without ketoacidosis. Data from Cases
and controls will be statistically analysed and then compared accordingly.
Expected Results: It is expected that decreased Sr. albumin will be associated with poor
prognosis in DKA patients. Sr. Albumin might be an indirect prognostic marker of diabetes
control due anabolic effect of Insulin, which affects both ketosis possibility and glucose
levels.
