Background: β-Thalassemia is a hereditary disease. The traditional method of treatment is blood transfusion and iron chelation therapy that may associated with many psychological complications. The aim of our study is to assess the intelligence quotient in patients with transfusion dependent and non-transfusion dependent βthalassemia. Patients and methods: This study included 36 patients with transfusiondependent thalassemia, 19 patients with non-transfusion-dependent thalassemia and 37 healthy subjects as a control group. Routine laboratory investigations were done for thalassemic patients according to international standards. Neuropsychologically tests were performed. Results: There is statistically significant difference between the studied groups regarding IQ score. On LSD comparison, the difference is significant between TDT and each of non-TDT and control groups where the lowest IQ prevailed in TDT group. There is statistically significant difference between the studied groups regarding IQ score. Low score presented only in TDT group (47.2%). High average occurred in 11.1% of non-TDT group and 13.9% of control group. Average score was present in 44.4%, 75% and 63.9% of those within TDT, non-TDT and control groups respectively. There is statistically significant difference between the studied groups regarding duration of transfusion (higher in non-TDT group).Conclusion: There is a difference in intelligence quotient and the degree cognitive affection between transfusion-dependent and non-transfusion-dependent β-thalassemia patients. Thus, IQ should be routinely assessed in B-thalassemia major patients for early detection of intellectual impairment and allow for appropriate management, in order to achieve a better life quality for this patients group.

Premature ejaculation (PE) is very common disease that affect more than 20% of menand the most common self-reported male sexual disorder. The two most relevant and evidence-based definitions of PE are those of International Society for Sexual Medicine (ISSM) and American Psychiatric Association. Both definitions agree on the chronic aspect of this disorder and the relatively short time interval between penetration and ejaculation (≤1min), the absence of voluntary control of ejaculation, and the negative consequences, such as distress in nearly all vaginal penetrations.

Background: Anatomical single bundle arthroscopic Anterior cruciate ligament (ACL) reconstruction has been the standard treatment for ACL injured patients. It is a minimally invasive procedure that allows the surgeon tounderstand the internal derangements of the knee joint. The purpose of Arthroscopic ACL reconstruction is to restore the normal kinematics of knee joint. The aim of the current study was evaluation of the results of anatomic single bundle anterior cruciate ligament reconstruction using hamstring tendon. Patients and methods: prospective clinical study was conducted in Orthopedic Department, Faculty of Medicine, Zagazig University, on 24 patients (18 males and 6 females with mean age 30.16±5.9) with chronic ACL tear treated with the hamstring tendon for reconstruction of the anterior cruciate ligament in all patients during the period from April 2020 to April 2021. Results: Grade of Lysholm score improved significantly postoperatively, where 17 patients were excellent, 5 patients were good, and 2 patients were fair. IKDC SCORE was improved from 45.83±7.46 preoperatively to 78.12±6.04 postoperatively. There were 19 patients without complication, 4 patients had superficial infection and 1 patient had deep infection. Conclusion: The anatomic anterior cruciate ligament reconstitution using hamstring tendon is more effective for reproducing the anatomy of the ACL and obtaining good clinical results, when it is done by accessory medial portal because the technique enables for a better view inside the knee and more obliquity of the reconstructed ACL when compared to the transtibial technique.

To assess role of MRI in detecting female infertility. Materials & Methods:Eighty- six women age ranged 20- 40 years were included in the study. All underwent MRI using 1.5 Tesla unit equipped with a 32 phased-array surface coil. Causes of female infertility was recorded. Results: Age group 20-30 years comprised of 50 and 31-40 years had 36 patients. Various causes of female infertility was PCOS in 12, tubal disease in 18, pelvic inflammatory disease in 10, endometriosis in 8, leiomyoma in 6, adenomyosis in 12 and endometrial polyps in 20 cases. A significant difference was observed (P< 0.05). Conclusion: MRI is an excellent non-invasive, radiation-free modality for the evaluation of female infertility; its superior soft-tissue contrast resolution and multiplanar evaluation generate exquisite anatomical details.

Primary Intracranial Ewing Sarcoma: Case series and review of literature

Ewing sarcoma, along with peripheral primitive neuroectodermal tumour, belongs to a tumour family that shares clinicopathologic and molecular genetic features, including the characteristic chromosomal translocation that results in the fusion opf EWS gene on 22q12 to either FLI1 gene on 11q24. In contrast , such translocations are not found in central primitive neuroectodermal tumours(cPNETs). Ewing sarcoma has only rarely been noted to primarily involve the central nervous system- extraoseus Ewing sarcoma(CNSESS). We report a case series of three patients with intracranial extension.

FAK and PCNA Double Immunohistochemical Expression among Different Oral Squamous Cell Carcinoma Grades

Objective: Focal adhesion kinase (FAK) is a tyrosine kinase that belongs to the focal adhesion complex that proved to have a serious role in cancer progression through its upregulation, while proliferating cell nuclear antigen (PCNA) is a proliferation indicating marker that involved in tumors behavior regarding invasiveness. Our aim is to assess the synchronization in the upregualtion of both markers through double immunostaining and their correlation with prognosis worsening in well differentiated and poorly differentiated oral squamous cell carcinoma (OSCC). Study design: A total number of 60 OSCC lesions distributed equally (n= 30) between the first group (well differentiated OSCC) & second group of (poorly differentiated OSCC). Double immunostaining with FAK &PCNA was carried out to determine their localization. Area percent measurement of FAK and nuclear count for PCNA were performed and analyzed. Results: Both area percent measurement of FAK and nuclear count of PCNA are higher in the poorly differentiated OSCC group than that of the well differentiated OSCC group, results showed statistically significant difference between the groups. Conclusion: Our results indicate that both FAK and PCNA are upregulated in poorly differentiated OSCC than in the well differentiated OSCC supporting their role in cancer aggressiveness & progression together with poor prognosis in higher grades of same tumor.

Background: Timolol, brimonidine and latanoprost are three different classes of drugs used in the treatment of glaucoma. These drugs act through different mechanisms to reduce the intraocular pressure (IOP). The present study aimed to evaluate the effectiveness of timolol 0.5%, brimonidine 0.2% and latanoprost 0.005% in treatment of glaucoma. Methods: This three arm parallel group randomized study was done at Narayana General Hospital & Modern Eye Care Hospital, Nellore. This study included 30 patients randomly allocated to three groups 1, 2, and 3 receiving treatments with timolol 0.5%, brimonidine 0.2% and latanoprost 0.005% respectively. Results: Total 10 patients were included in each group (1, 2, 3). The baseline characteristics were comparable between the groups. The group 1 patients showed significant decrease in IOP on day 3 of treatment, in comparison to baseline IOP. The same was observed with other groups also. In comparison of mean reduction of IOP between groups, group 3 patients showed significantly more reduction in IOP. Conclusion: Latanoprost has shown more efficacy in comparison to timolol and brimonidine in reducing the IOP.

EFFECT OF CRYOTHERAPY ON HYPERTROPHIC SCAR

Background: Hypertrophic scar formation is a major clinical problem in the developing and industrialized worlds, Cryotherapy is the local or general use of low temperatures in medical treatment. Purpose: The purpose of the study was to evaluate the therapeutic effect of cryotherpy in treating hypertrophic scar. Method: Sixty patients who had hypertrophic scar participated in this study. Their ages ranged from 20 to 40 years. The participants were selected from Deraya university outpatient clinic and EL Minia University Hospital. They were randomly distributed into two equal groups. Group A (control group): This group received routine medical treatment and routine physical therapy including (stretching Exercises, Positioning and splinting, Pressure Therapy and Massage). Group B (study group): This group received cryotherapy for 10 minutes at -14 degree, 2 sessions per week, for 10 weeks in addition to routine medical and physical therapy treatment. Measurements were done using MAPS and were recorded in the first visit as a base line and after 3 months of treatment. Results: There was a statistically significant difference between groups in the study group compared with the control group regarding post treatment data of surface, height, thickness, color, pigmentation and the total score of MAPS where p value equals (0.005, 0.0001, 0.0005, 0.0021, 0.00016, < 0.00001) respectively. Conclusion: Cryotherapy had significant effect in improving the outcome and treating hyper trophic scar.

Thalidomide is better than Rapamycin in prevention of neovascularization after laser photocoagulation in dorsal window chamber rat model

Objectives: -This study examines the effect of two different antiangiogenic drugs (rapamycin and thalidomide) in prevention of neovascularization when locally used, after laser coagulation through dorsal skinfold window chamber rat model. - Determine what is the most effective antiangiogenic drug when used locally and its potential side effects. Methodology: The study includes 3 groups, each group contains 10 rats Thalidomide, Rapamycin and control group (laser group). Blood vessels in the dorsal window chambers implanted on dorsal skin of albino rats were irradiated by Diode laser pulses until coagulation occurs. Blood flow and vessels structure were documented with high definition camera to evaluate photocoagulation and reperfusion. Treatment groups received the treatment drug which was injected to the widow’s medial side, every 3 days’ intervals for 2 weeks after laser irradiation. Results: Control group, 26 out of 28 photocoagulated blood vessels reperfused within 5-12 days with reperfusion rate about 93%, in Rapamycin group the reperfusion rate was about 46%. We also found that the reperfusion rate in Thalidomide group was about 8% which was the lowest reperfusion rate in all groups. Conclusions: Thalidomide group reperfusion rate was about 8% which was the lowest reperfusion rate in all groups. Thalidomide is more effective than Rapamycin, and it`s antiangiogenic effect lasts for weeks with no side effects detected with local use.

A Performance Study of ML Models and Neural Networks for Detection of Parkinson Disease using Dysarthria Symptoms

Parkinson Disease (PD) is brain disorder that affects the central nervous system that results in damage of nerve cells causing dopamine to drop. PD has a severe effect on vocal features termed as Dysarthria symptoms including varied pitch, extended pauses, monotonous and speaking slowly or with a slur. In this paper, a dataset containing various vocal features are taken as input to analyze the performance of various Machine Learning algorithms including Naive Bayes, Random Forest Classifier, Support Vector Machines (SVM), Linear Regression, K Nearest Neighbor (KNN) and Neural Networks such as ANN and LSTM. The best classification accuracy was obtained by ANN around 90.00%.

How Behavioral Change Wheel Strength Self-Control in Health Behaviors?

The purpose of this study is to develop a planned intervention in the health care sector in Bahrain to improve the self-control of healthcare employees by using the Behavioral Change Wheel (BCW) anew method for characterizing and designing behavior change. Hence, the study applies a longitudinal methodology to the healthcare sector in the Kingdom of Bahrain for a sample of 64 participants from November 2019 to January 2020. The procedures of this planned change task occupy three stages, based on Kurt Lewin change field theory: Stage 1 (pre-assessment of employees’self-control using the Brief Self Control Scale (BSCS) questionnaire);Stage 2: planned intervention employing the BCW as a new method of change): and Stage 3 (post assessment by repeating the self-control measurement, and comparing the new results with the previous ones). The findings reveal that the 13Items of the Brief Self-Control Scale (BSCS) provided different results over the three stages of the study. In Stage 1 (pre-assessment) the level of self-control level was low, but had improved in stage 3 (post-assessment). This is original research that adds to the body of knowledge by developing and validating a new model in changing human behavior, i.e. the change behavior stages (CBS) model. Practically, this study is the only one of its kind to apply the Brief Self Control Scale (BSCS) and the BCW in the health sector. The BCW has been alsoconfirmed as a “valid” and “reliable” instrument that can be used by future researchers to improve human behavior in organizations.
