Abstract

Background:Fistula-in-ano is one of the commonest benign colorectal diseases, and it is defined as abnormally epithelized tract connecting two surfaces, usually perianal skin and rectal mucosa. Fistula-in-ano was previously treated with fistulectomy and Fistulotomy. In recent era LIFT, glue and Seton are other treatment modalities. Objective: To compare different modality of treatment for fistula in ano. Methods: A randomized study on various management was conducted at ESIC & RC Gulbarga, between January 2017 and January 2022 to compare various management of fistula in ano. Results: A total of 100 patients were taken in study. Male to female ratio was 4;1 {80%;20%}. 90% of patient had < 1 year’s symptom duration. All patients had discharge as main symptoms followed by pain around 80% cases, other symptoms include bleeding swelling and itching. Post operatively Fistulotomy has highest number of complication in the form of fever, infection whereas Seton required multiple setting. Most number of cases has pain as complication in our series. LIFT and glue were having least number of post op complication rate and symptoms. Most of these patient were discharged within 3 days and around 68% patients did not  stay more than 10 days. Conclusion: We want to conclude that LIFT procedure and Fistulotomy were acceptable procedures for simple, uncomplicated low lying and high lying fistula and for complex and recurrent fistula needs LIFT with Seton or Seton multiple setting, glue is alternative in recurrent fistula with failed surgery
