Pneumonectomy have a greater impact on lung function causes several pulmonary complications secondary to it. However, there are minimal to no evidence present which justify the effectiveness of chest physical therapy rehabilitation in context with long term follow up as well as reduction of complication in several pneumonectomy patients. The given cases provide effectiveness of early and routine physical therapy in order to avoid unnecessary post-operative complication as well as importance and benefits of physical therapy immediate after surgery.
The series consist of two cases. First is 45 year old female operated for carcinoma with pneumonectomy procedure. Physical examination reveals pain and discomfort immediate after post-op. Second case shows a 53 year old male patient operated for Tuberculosis with pneumonectomy procedure and after 5 years, physical examination reveals pain, breathlessness, respiratory muscle wasting and weakness as well as abnormal postural traits.
Diagnosis, Intervention and outcome:
Both the cases were of pneumonectomy but with different etiologies, one secondary to squamous cell carcinoma and other secondary to tuberculosis. Intervention included protocol based and evidence based approach with different modalities, exercise and education program and use of manual and mechanical devices over the course of duration in progressive manner. Outcomes are gross improvement in strength of muscle groups on
MRC, 6 MWT was use for endurance and ambulation and distance was increase above MCD in both the cases, Functional Independence Measure shows improvement in functional independency and RAND 36 health survey 1.0 reveals improved Quality of Life.
Physiotherapy rehabilitation programmed, evidence based and routine physical therapy rehabilitation has been found effective in maintaining functional independence, better quality of life and the prevention of unnecessary post-opt complications. Also, report signifies how the ignorance can lead to long term complications, which were otherwise preventable.
