Study of clinical, bacteriological and radiological features of Community Acquired Pneumonia.

Abstract

Pneumonia is a common illness affecting approximately 450 million people a year and occurring in all parts of the world. It is a major cause of death among all age groups resulting in 4 million deaths (7% of the world's total death) yearly. This study was performed to understand the mode of presentation, its clinical features, bacteriological and radiological features for the early detection of disease, the causative agent and to study its complications. Prospective observational study was conducted in Department of Pulmonary Medicine, Rajiv Gandhi Institute of Medical Sciences Adilabad, Telangana. 50 patients diagnosed as Community Acquired Pneumonia (CAP) were included in study. All the patients are subjected for detailed clinical examination to make a provisional diagnosis of Community Acquired Pneumonia (CAP). Sputum for Gram stain and Culture, sputum AFB are done. Blood for WBC Count and Differential Count are done .Chest X-ray done to know the Site of consolidation. Statistical analysis was done. The commonest presenting symptoms are fever (100%), cough (100%), expectoration (100%), other symptoms include dyspnoea (62%) and chest pain (64%). The sputum examination showed 72% Grams positive, 24% Gram negative and 4.0% are mixed. Sputum culture showed 48% streptococcal, 24% staphylococcus aureus, 4% pseudomonas, 16% Klebsiella, 4 % non commensal E.coli and 4% are mixed. Early diagnosis, especially in high risk patients such as patients with COPD, Diabetes, Alcoholics, Smokers with early initiation of antibiotics based on the prevalence of organisms in that community can help reduce the morbidity and mortality in Community Acquired Pneumonia
